We are looking to appoint a Social Worker to join the newly developed whole life disability service. The service is structured to work with individuals who are 0-25yrs and 25yrs and over. This vacancy is within the 0-25 element of the team. The successful candidate will contribute to the team’s multi-agency approach to providing a seamless health, social care and well-being service for disabled people of all ages and their families. The Social Services and Wellbeing (Wales) Act (2014) provides a legislative framework to ensure that individuals are supported to achieve positive outcomes at all stages of their lives. You will work with individuals so that they are enabled to identify their own personal outcomes, and how they can achieve those outcomes. Your work will focus on the individual and their support networks including families to promote independence, ensure the individual has a stronger voice in decision making and has more control in their lives.
To be the right person for this role, you will have excellent communication skills, proven experience of working as part of a team, previous experience of working with children and / or vulnerable adults and the ability to respond appropriately to the needs of disabled children and adults whilst working within statutory guidelines, policies and standards.
In order to be considered for this post you must be a registered Social Worker.
