[CLOSING DATE: Fri, 22 Jun 2018] We have a vacancy for an enthusiastic motivated individual to join Wrexham’s dynamic Leaving Care Team. You will have experience of working with young people either individually or in groups. You will have knowledge of the needs of young people leaving care and the barriers they face as they move from the care of the local authority into adulthood and independence in accordance with Social Services and Wellbeing Act 2014.
You will hold a caseload of young people aged 16 to 25 and will be expected to support them into adulthood providing a range of practical and emotional support. This will involve advocacy, negotiation and working with other agencies and colleagues to ensure young people’s needs are met. You will be committed to supporting young people to reach their potential.
You will need to have excellent communication skills and the ability to form positive, sustainable relationships with a diverse group of young people.
You must be able to work under pressure, be sensitive to the confidential nature of the work and have an understanding and commitment to the promotion of equal opportunities and anti oppressive practice.
You will be expected to work independently and as a member of a team. To support you we will offer continuous professional development including supervision and support and ongoing training.
