[CLOSING DATE: Fri, 19 Apr 2019] An opportunity has arisen in Primary Mental Health Support Services to join a busy and supportive team based in Wrexham.
The successful candidate whilst working closely with a multidisciplinary team will receive referrals directly from the GP's and provide short
interventions for individuals with mild to moderate mental illness. You will sign post service users to other statutory agencies and a range of third
sector organisations.
The successful candidate will be expected to undertake assessments, risk assessments and risk management plans. There is also the opportunity
to co-facilitate therapeutic groups. We are looking for a good team player who has excellent organisational skills and the ability to prioritise their
time effectively.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
