Community Disability Team
A unique opportunity to join the Community Disability Team as a Community Care Worker has arisen for a suitably qualified person.
We are looking for an enthusiastic person who will be able to contribute towards the improvement and development of the team and person centred service delivery. You will be expected to work directly with both vulnerable young adults and young adults with learning disabilities, to establish need, develop enablement and outcome focused support plans, care plans and support Social Workers to deliver ‘What Matters’ ( Social Services and Wellbeing Act 2014)
As part of an integrated specialist team you will be expected to work as part of a multi-disciplinary team to provide comprehensive assessment and review of a small allocated caseload. The post holder will receive professional and operational line management by a senior Social Worker in the team and be responsible to the Team Manager.
