Exciting opportunities exist within the
Team for Older People
3 x 37 hour posts – temporary for 9 – 12 months£23,398 - £30,785 per annum
Applications are sought from Qualified Social Workers (level 2/3) for the Social Worker posts within the Team for Older People.
The successful candidates will have a wide range of experience and Knowledge and will need to demonstrate an interest in developing social care for older people with mental health.
The Team for Older People works closely with partners agencies to support and promote timely Hospital discharge and provide appropriate interventions for older people to ensure they have every opportunity to remain independent within the Community.
Experience of Multidisciplinary working would be an advantage. The potholder will be expected to operate within a multidisciplinary framework with a wide range of agencies, carers, support providers and third sector agencies. The ability to demonstrate an understanding of how legislation is shaping the landscape of Social Care in Wales is essential. It is desirable that the post holder has some experience of working with adults who have physical and mental health difficulties.
An understanding of Intermediate care would also be desirable.
