Ty Derbyn, Croesnewydd Rd, Wrexham. Ll137TD The main duties of this post will be the care coordination of service users within the group/supported living area of the Recovery service, in line with the mental health (Wales) Measure 2010. You will be required to carry out needs assessments and robust risk assessment and risk management which will also direct the team within their role. Working within the umbrella of BCUHB you will be required to adhere to systems and policies and procedures of both WCBC and BCUHB. This includes undertaking file audits on share point ensuring this remains up to date. You will be required to work as part of a multidisciplinary team which includes, CPN, OT, RC home leaders and project workers. This includes supporting and delivering training to support staff and taking an active role in service user involvement. You will also be required to maintain accurate and relevant case records, reports and other documents within the framework laid down by the department sand team management, including the preparation of specialist reposts as required. As a registered service with CSSIW you will also be required to undertake SDP reviews alongside home leaders / project workers and work within the parameters of the care standards Act 2000. You will need at least 2 years post qualifying experience in a mental health setting as a care coordinator.
